Camper Forms & Information

Measures for Success for Campers  Some things to consider before sending your child to camp; here are some essential camper functions for a positive camp experience.

Camper Physician Release   You must have this form completed by your camper’s primary care physician or endocrinologist before attending camp. Please note that with our new registration platform via CampDocs it is the guardian's responsibility to insure that the camper's Physician Release has been uploaded to the participant profile. See the camper's CampDocs profile for additional instructions or email stix@stixdiabetes.org with any questions about this process. 

Camper Expectations and Code of Conduct  These are the expectations for all campers in attendance at camp. Failure to comply with the Camper Expectations may result in dismissal from camp

Camper Supplies to Pack  Please review this list to be sure that you have all the necessities before coming to camp.

Continuous Glucose Monitor (CGM) Agreement  If your camper plans to bring a CGM to camp, be sure to review this agreement. You can bring a signed copy with you or we will provide you with one to sign at check-in.

Dress Code  Be sure to review this dress code when packing.

STIX Diabetes Programs Campership Policy  If you are concerned about your ability to afford camp, please review our campership policy. We will do everything we can to ensure that your camper can attend camp!

Homesickness Advice for Parents & Campers  Please review this information with your camper, especially if this is your camper’s first year at camp or first time away from home.

Bedwetting Information  If your camper has a recent history of bedwetting, please review this document to help their camp experience go as well as possible.

Camper Releases  These are the releases for STIX Diabetes Programs that you signed during registration.

YMCA Camp Reed Release  This is the hold harmless agreement from Camp Reed/YMCA of the Inland Northwest that you signed during registration.

 

Volunteer Forms & Information

Volunteer Physician Release   You must have this form completed by your primary care physician or endocrinologist before volunteering at camp. Please note that with our new registration platform via CampDocs it is YOUR responsibility to insure that your Physician Release has been uploaded to your participant profile. See your CampDocs profile for additional instructions or email stix@stixdiabetes.org with any questions about this process. 

Volunteer Code of Conduct  During the volunteer selection and training process, STIX Diabetes Programs will make every effort to provide volunteers with policies that govern our living and working in a healthy and safe environment. This policy must be understood and agreed to by all volunteers and is signed during the application process. Violations of this code may result in dismissal.

STIX Diabetes Programs Child Abuse Prevention Policies and Reporting Procedures  STIX Diabetes Programs will make every effort to provide volunteers with education and policies that prevent and protect both campers and volunteers from abuse and neglect.

Children of Volunteers Policy  If you will be volunteering at camp while your child is a camper, please review this policy to ensure that you and your camper have a great week at camp.

Minor Volunteer Guardian Authorization  This form is required for all minor volunteers who will be attending Camp STIX without a legal guardian present for the duration of their volunteering. (excludes Junior Counselors)

YMCA Camp Reed Release  This is the hold harmless agreement from Camp Reed/YMCA of the Inland Northwest that you sign during the application process.

STIX Diabetes Programs HbA1C Guideline  This is the set of guidelines that you sign as having read during the application process. In order to accommodate for this practice, please note when the Volunteer Physician Release is due.

Out-of-state RN Notification  This is a letter that out-of-state nurses can use to let the Washington State Board of Health know they will be volunteering in WA. A copy of this letter must be sent at least 10 days prior to camp to nursingpractice@doh.wa.gov

Liability Insurance for Providers This is a link to the liability insurance company that will cover providers for malpractice coverage while volunteering at camp if they are not already covered by their employer. 

MASH Manual  All MASH staff will be required to sign a document indicating you have read and understand this MASH manual, and agree to abide by it.
